Stories in Stone Brick Campaign

Deadline to order bricks: June 19th, 2009

Don't miss this opportunity!

The Stories in Stone program offers a rare opportunity to celebrate life with a lasting symbol of love and friendship.  For a limited time, friends of Lake Hospital System can honor those who have touched their lives by purchasing an inscribed tribute stone.  The paving bricks will be installed in the Courtyard of the New TriPoint Medical Center - one of the most important investments in the health and vitality of our region.

Through the Stories in Stone program, we are offering donors an occasion to support this patient- and family-centered hospital, at the same time receiving tangible recognition for their generosity.  You may choose to have your name engraved in the brick, have it inscribed in memory or in honor of another individual, or use it as an opportunity to leave a personalized message.  Your gift will make a lasting impression for generations to come.

Two Brick Sizes are Available

For a tax-deductible gift of $250, an 8" x 8" engraved stone will be installed in TriPoint Medical Center's Courtyard with your personalized message.  Each 8" x 8" brick can be engraved with up to three lines of lettering with a maximum of 14 characters per line, including spaces and punctuation.

Building for the Future - Capital Campaign

Lake Hospital Foundation invites you to participate in an historical Capital Campaign that will raise $10 million to support the construction of TriPoint Medical Center, our new hospital, which will be located in Concord Township. The 300,000 square foot state-of-the-art digital hospital will include 119 private patient rooms overlooking a natural setting and a 28-bed emergency department.

TriPoint Medical Center has earned the noble distinction as one of 35 health care facilities nationwide to be selected by The Center for Health Design to participate in its Pebble Project. The Pebble Project is a joint research effort between The Center for Health Design and several healthcare organizations throughout the country who believe there is a correlation between facility design and an improvement in health care. The vision for TriPoint Medical Center has always centered on providing a healing environment; being a part of the Pebble Project will ensure that we achieve our goal.

How Your Donated Dollars Help

Through your generous annual gifts, Lake Hospital Foundation is able to help provide superb medical care through advanced technology, cutting edge clinical practices, and compassionate patient care. Below are examples of where your donations to Lake Hospital Foundation are applied:

WOMEN’S HEALTH

Lake Hospital System has accredited mammography services at five locations including a digital mammography unit at LakeWest.   For those who have had an abnormal mammogram, Lake Hospital Foundation supports a uniquely effective Breast Health program. A coordinator works individually to assist the patient in navigating the process which includes: understanding diagnosis and treatment options, assisting with informed decision making, and coaching the patient as well as family members.   Gift support of this uniquely valuable program in the form of cash, stock or a bequest secures this service for patients and families.

INTEGRATIVE MEDICINE

Pause, Reflect, Heal

Lake Hospital System is introducing an Integrative Medicine program that combines conventional medicine with ancient wisdom and complementary therapies to treat the whole person.   The selected therapies are evidenced based and include healing touch, medical massage, music therapy and pet therapy.   The program was launched on the oncology and orthopedic units at LakeWest - gift support will help expand this program throughout Lake Hospital System.

DIABETES EDUCATION

Lake Hospital Foundation wishes to enhance support provided to diabetic education initiatives.   Locally there is a high incidence of diabetes, a disease that ultimately leads to high blood sugar levels and over a long period of time can damage kidneys, nerves, blood vessels and the retina of the eye.  Risk factors for developing diabetes include high blood pressure, high-fat diet, high alcohol intake, sedentary lifestyle, and obesity or being overweight.   Age become a risk factor for those 45 and older.    Diabetes is believed to have a strong genetic link with African Americans, Native Americans, Hispanic Americans, and Japanese Americans at risk.   Financial support of education efforts helps individuals manage this chronic disease.

CANCER CARE

The Lake/University Ireland Cancer Center in Mentor, Ohio provides multifaceted cancer care with both medical oncology and radiation therapy.   The site has a linear accelerator for radiation therapy and day beds for chemotherapy. Opportunities exist for patients to participate in clinical trials.   The Center is a joint venture with the University Hospitals of Cleveland Ireland Cancer Center. With gifts provided from many sources, Lake Hospital Foundation has helped to provide massage therapy, at no cost, to cancer patients.   Massage therapy reduces swelling and other uncomfortable side effects of treatments.   A comprehensive outpatient Pallative Care program will be introduced should adequate funding be obtained.   Please support cancer services with a gift of cash, stock or a bequest.

ENDOWMENT

Creating a Legacy of Health

Create a legacy at Lake Hospital Foundation through estate planning and enhance the health of your family and the community for generations to come.   Careful planning today makes it possible for almost anyone to give more in the future.   For information on how to leave a lasting legacy please contact Sunny Masters (440) 354-1860 sunny.masters@LHS.net   or

GIFTING

Think of unrestricted gifts as the key that opens a locked door … think of unrestricted gifts as the bridge over the river that gets us from where we are today to where we need to be tomorrow.   Unrestricted contributions help solve the most pressing needs; unrestricted contributions create opportunities. In addition to participation in the annual Glitz and/or the annual Golf Tournament, please consider an unrestricted annual gift.

Thank You!

Ways To Contribute

There are a number of ways to support Lake Hospital System in its mission to enhance the health status of the community. Deciding how to give is a personal decision. In some cases a tax adviser or attorney should be consulted. Following are examples of ways to contribute.

Gifts Of Cash — This is the most typical way to contribute and includes donations that are charged to Visa or MasterCard. The Foundation can process cash as a one-time gift or as payment on a pledge.

Gifts Of Stock — Gifts of stock usually afford the donor a tax benefit and can also result in a larger gift. Notify the Foundation office of your intent to contribute stock. Stock gifts can be recorded as a donor’s annual gift or as payment of a pledge.

Name Us In Your Will — Consult your attorney and prepare your will. It is one of your most important documents. Providing for Lake Hospital System in your will is a lasting tribute. Speak with your attorney.

Use Life Insurance As The Gift Vehicle — You can name the hospital as a beneficiary of your policy.

Other Planned Gifts — These include trusts, unitrusts, charitable remainder trusts, etc. These are vehicles that should be discussed with your estate planner, attorney, or financial adviser. The Foundation is available, at no cost, to provide information to a donor’s experts in execution of the vehicle.

Gifts Of Real Estate — Contact the Foundation office if you would like to make a gift of real estate or another tangible asset.   (Acceptance is by Board approval.)

Gifts Given in Tribute — You can pay tribute to a loved one; celebrate an anniversary, birthday, or graduation; or honor your physician with your donation. Send your check, along with the name and address of the person you would like the Foundation to notify. A letter will be sent that tells the honoree (or a survivor of the honoree if the gift is a memorial), of your thoughtful gesture. The amount of the donation is not mentioned.
